Please fill in the form below completely. In 300 words or fewer, tell us about your experience. Address how your nurse demonstrated compassion, strong clinical skills, excellent communication, trust worthiness and patient advocacy.

Nominations must be submitted by someone who has been taken care of by the nurse, by a relative or close friend of a patient the nurse took care of, or by the nurse's colleagues. Entries are due March 20, 2015. The special Salute to Nurses section will publish Sunday, May 3, 2015 in The Boston Globe.
